You must be over 16 years old and have one of the following: A confirmed diagnosis of Vitamin B12 deficiency you will need to bring proof of this on the day of your appointment, and it must be dated within three months of your appointment Bariatric surgery and need Vitamin B12 to prevent a deficiency you will need to bring proof of this on the day of your appointment, and it must be dated within three months of your appointment Maintenance therapy of Vitamin B12 deficiency after having received initial Vitamin B12 treatment for pernicious anaemia you will need to bring proof of this on the day of your appointment If you are pregnant or breastfeeding, you cannot have this treatment
